Immunization requirements for Broward County
The following is a guide to immunizations required at Broward County Public Schools:
Forms: The proper form needed to document a student's immunization history is the Department of health Form 680.
Minimum Required Immunizations:
Vaccine Dose
- Diphtheria Tetanus Pertussis (DTP) 5 doses
If 4th dose was given on or after the 4th birthday, only 4 doses are required
- Oral Polio Vaccine (OPV)
4 doses If 3rd dose was given on or after the 4th birthday, only 3 doses are required
- Measles, Mumps, Rubella (MMR)
2 doses of Measles K-12th grade (MMR preferred)
Additional vaccine requirements for selected grades:
Hepatitis B (series of 3 shots) PreK - 12
Tetanus/Diphtheria (Td) booster 7-12
Varicella Vaccine (Chicken Pox) PreK, K , 1st , 2nd, 3rd, 4th
Students may be exempted from the required immunizations for religious or other reasons - parents should contact the Health Education Services Department directly at 754-321-2272 for more information.
